Process

• Must be 21 years of age or above, unless given permission from parents or guardian.
• Must be a resident of Vermont with a Vermont identification card as proof or residency.

Patient must be suffering from any qualifying conditions.

Written certification or recommendation of qualifying conditions from a registered physician

• Must complete application form online to be register and to legally use medical marijuana
• Obtain all required documentation and complete all required forms.
• Must pay required registration fees to DPS
