How long does it take?
If a patient’s application is approved, within thirty days the Marijuana Registry Identification Card should be received by the patient. If the application is not approved, the patient will be notified through writing and be advised of the right to appeal.
How do I qualify?
- Patient must have any of the qualifying conditions
- Must have completed Registered Patient Application Form
- Must have a Physician’s Medical Verification Form signed by the patient’s doctor
- If patient is below 18 years old, the application should be signed by both parents or guardians
